A large academic medical center sought to improve interdisciplinary communication and progression of care by assigning patients to units based on clinical needs, nursing competency and provider staffing. This session will present the process to assess service line census with bed allocation and the implementation processes to successfully cohort patients. Lessons learned during implementation will be shared as well as the family of metrics that were followed to support and monitor efficacy.
